Eligibility Criteria for Uttar Pradesh National Family Benefit Scheme
  • Families living below the poverty line can do the rounds.
  • The applicant should be 18 years to 60 years of age.
  • Income of the applicant’s family of urban area applicants should not exceed Rs 56,450.
  • Household income of the applicant of rural area should not exceed Rs. 46,080.
Documents Required
  • Photo of the applicant.
  • Applicant’s Age Certificate
  • Income certificate of the applicant’s family, income certificate, salaries or salary slip.
  • Death of family head or certificate of death of family earning member, meaning of head, the person who used to make money.
  • Scanned copy of CBC Bank Account Details and Bank Pass Book
  • Copy of print out of application form and computer generated receipt of application.
  • Hard copy / print out / photostat of all these documents should be submitted in the time of submission by the applicant to the District Office. Apart from this, all these documents should be verified by the applicant.
