State Health Card Uttar Pradesh Print Application Form State Health Card Apply online Edit Application Verificatin Process Contact Details The state government of Uttar Pradesh is accepting online application for State Health Card UP. Under the scheme, government will provides cashless treatment of emergency and incurable diseases for state government employee. The scheme is running under Pandit Deendayal Upadhyay State Employee Cashless Medical Scheme. 

About the Scheme

Under the scheme, the facility of treating casual and incurable diseases to state employees of Uttar Pradesh, pensioners and their family members will be available in government hospitals and other medical institutions in addition to CGHS contracted private hospitals.

Apply Procedure for State Health Card

  • First go the official website at http://upsects.in
  • Drag Your Mouse Pointer to Employer/Pensioner Gateway
  • Now click on “Apply state Health Card”
  • Here you will find a notification read carefully the instruction and click close
  • Now an application form will be appear
  • Fill all the details
  • Fill your employee status either employee or Pensioner
  • Select department name and district
  • Fill code, office name, applicant name and DOB
  • Select Post and fill basic pay mobile number, Email id, Aadhar number and address for correspondence
  • Upload picture in jpg/jpeg of 20 kb

Form Verification Process

The verification of the form of state employees andpensioners will be done by the concerned withdrawal officer and treasurerrespectively. After verification of the beneficiaries’ online form, provisionhas been made to print “State Health Card” (Paper Card) at thebeneficiary level, with the help of which the cashless medical facility will beavailable to the beneficiary.

Add/Edit Application

  • If the applicant wants to add or edit in their application then go to the Add/Edit section of the web page
  • After clicking on the Add/Edit section a new page will be open
  • Fill applicant Aadhar number and get OTP for further edit application

How to Download Application

  • Go to the Download Application Section or go directly by following link
  • http://upsects.in/public/chkentr.aspx
  • Provide your Aadhar number
  • Click on send OTP
  • After it fill the OTP that is send in your registered mobile number

How to Print

  • Click on the Print State Health Card or directly go by following link
  • http://upsects.in/public/empcard.aspx
  • Enter Aadhar Number and click on send OTP
  • Now your card will be display
  • Click on the print your Aadhar Number
